Referring

We cannot take self-referrals to the service but accept referrals from health professionals.

If you are a dietitian, doctor, nurse or social worker and you would like to make a referral, the form and information to assist completion, can be downloaded using the link below and either faxed to 020 7359 8169 or emailed to info@foodchain.org.uk

It is very important that you assess the client according to our criteria and that you complete the form fully. We rely on your support to ensure that only people who need the service receive it.

When we have received the referral, we will contact you to advise that it has been received and if the client meets the criteria and when we will be able to take them onto service.
