Re-referral

As we have limited resources and waiting lists for our services, after 6 months we will contact you for a re-referral.

We will ask you to assess how the service has helped your client, if they still meet the criteria and if the service should continue.

We ask that you help us with the re-referral process by completing the form fully and carefully, assessing your client as to their need for service.

If we do not receive a re-referral it may be necessary to suspend the service for your client.
